  • We will send your information to you through Spruce.


Requesting your medical records to be sent to someone else

  • Download and fill out the Release of Information form below
  • Send it to your Care Team via Spruce
  • We will send the information to whoever you have requested.
  • If you are having trouble downloading the form below, let your Care Team know and we will manually send you the form via Spruce.
