About Forms

Your Forms section is where you can review and complete any forms your Care Team has requested for you to complete. This includes your intake forms, consent forms, photo ID, and other information required for sign-up. 

You can view both completed and incomplete forms under the “All forms” tab. You can also view only what you need to complete under “Not started/Incomplete” and everything you have already completed under the tab “Completed/Skipped”. 

Completed forms

If a form has been completed, a green “Completed” button will appear next to the form along with the date of completion. 

Incomplete forms

If a form needs to be completed, an orange “Not started” button will appear next to the form along with the date the form was requested. You can fill out the form directly from here by clicking the blue “Fill Out” button next to “Not started”
