Click “Plume Insurance Intake” when prompted by your Care Team member to begin completing the form

Let us know the name of your insurance plan, including the Member ID and Group Number according to your active insurance card. Please also include the home address associated with the insurance policy. 

The last step is to take a photo of the front and back of your insurance card and submit your form. You will have the opportunity to inform your Care Team of anything related to your insurance or care we should know ahead of time. 

Submit your completed form. From here your inquiry will be submitted for eligibility review. We will get back to you in 2-3 business days letting you know if your insurance has been accepted
