If your unit’s leadership is requiring a letter of support and/or requesting information regarding your transition, we can provide that for you! Message your Care Team letting us know the following information and any additional documentation needed: 

• Date (month and year) you started GAHT (gender-affirming hormone therapy, sometimes called HRT) 
• Date (month and year) you started socially identifying as trans or non-binary
• The full name you would like to adopt as your legal name (First, Middle and Last)
• Gender marker you would like to use as your legal gender marker
  
• Gender marker currently on your legal documents, such as your passport or driver’s license
  
• The date you need the letter completed by
