Plume Clinic has made the difficult decision to end our $32/month insurance-based membership, effective December 31st, 2025. By December 31, 2025, all patients with an insurance-based membership will need to either find a different in-network provider or choose to switch to Plume’s self-pay option to continue their care.
We know this news is disappointing, and we hope to limit any disruptions to your care. While we can no longer offer insurance-based membership options at this time, we are still committed to providing gender-affirming care to the trans and gender-diverse community.
Can I still receive care with Plume Clinic?
Yes! You can receive care with your insurance-based membership through 12/31/25.
If you choose to continue your care with Plume after 12/31/25, you will need to switch to our self-pay membership, which includes the same services and access to your Plume Clinic Care Team.
Self-pay members can choose between monthly or yearly billing plans (37% savings when paid annually).
If you prefer to use your insurance, you will need to find a different in-network provider.

Some states require members to pay for labs out of pocket due to state laws around lab billing, such as New York, New Jersey, Rhode Island, and North Dakota. To offset this cost, we offer a reduced rate for these states.
When does my insurance-based membership end?
Your current insurance membership will remain active through December 31, 2025. If you choose to switch to a self-pay plan, your new plan will go into effect on January 1st, 2026. You will see the price change on your January invoice. If you choose to find an in-network provider outside Plume and cancel, your insurance-based membership will end on December 31st, 2025.
I’m worried I will not be able to find an in-network provider in time, and my prescription might run out. What should I do?
If you need help finding an in-network provider or are concerned about your prescription running out, please reach out to your Care Team via Spruce. We’re happy to help you and, if needed, provide you with a temporary bridge prescription (please note this is subject to prescription restrictions and your provider’s discretion).
How do I switch to the self-pay plan?
- Message your Care Team in Spruce before December 31st, 2025, and tell us you want to switch*.
- We’ll update your membership using your existing billing information.
- Once updated, you’ll receive a confirmation message in Spruce.

What if I choose to cancel?
If you decide to cancel your membership, you may continue to access care and utilize your membership until December 31st, 2025.
Your Plume Clinic provider will work with you to support access to your medication while you set up care elsewhere. This support is dependent on prescription restrictions and your provider’s discretion. For questions about your care, please message your Care Team in Spruce. You’ll also receive a free 6-month Community Membership, giving you access to Prism for peer support groups, workshops, and transition resources.
Will this affect current self-pay or Community Memberships?
No. If you’re already enrolled in Plume Clinic’s self-pay Full Membership or Community Membership, nothing is changing.
Is Plume Clinic still providing gender-affirming care?
Absolutely. Plume Clinic remains fully committed to providing gender-affirming hormone therapy and community support for trans and gender-diverse adults. Only the insurance-based membership is ending. Our clinical care and services continue without interruption.
What happens if I don’t take action?
If you don’t respond by December 31st, 2025, your insurance-based membership will automatically cancel. To continue your care, you’ll need to either find another in-network provider or switch to a self-pay plan with Plume Clinic. If you plan to switch to self-pay or need support managing your medication during this transition, please notify us by December 31st, 2025 by messaging your Care Team in Spruce.
Is Plume Clinic closing?
No, Plume Clinic is not closing. This decision helps us strengthen our self-pay model & Community Membership.
Is this decision related to the current political climate?
Over the past few years, the trans community—including us at Plume Clinic—has had to navigate a challenging environment. However, this decision is not politically motivated. It’s based on the need to maintain a sustainable service that ensures Plume Clinic can continue providing care and supporting the trans community for years to come. We remain committed to advocating for access to gender-affirming care and supporting our members.
